Who Can Apply For an Ontario Birth Certificate?

This site is available for use by the legal profession and general public to obtain an Ontario Government approved and official certificates of birth issued by the government agency as an independent authorized agent.

Original or replacement birth certificates may be released to:

  • The person who is the subject of the birth certificate and is at least 13 years old; or
  • A parent of the person who is subject to the certificate, if that person is less than 19 years of age or incapable

If you are applying for an Ontario birth certificate to support an application for other identification such as a Passport, Driver’s License etc., please check with the issuing agency to ensure you obtain the correct certificate. Changes cannot be made once an order has been placed.

Search Letter– $65.00 – ($15 extra for each additional name searched)

Fees include all taxes, search, shipping and processing charges.

If no record of the birth is found by the Office of the Registrar General, fees will be applied to the search cost and will not be refunded by the Registrar General.

Newborns – An Ontario birth certificate will be issued upon completion of the birth registration. The registration takes approximately 16 weeks from the date of birth. Rush service is not available for newborns born within the last 16 weeks. Please contact the Registrar’s Office at 1-800-461-2156 if you require a certificate for a newborn on a rush basis.

Births Prior to 1930 – Births prior to 1930, may not be in electronic format. When applying for an Ontario birth certificate for a birth prior to 1930, it can take up to an extra 6-8 weeks for the registration to be converted into electronic format before processing can be completed.

Short Form – Ontario Birth Certificate

The short form contains the individual’s name, date of birth, certificate number, birthplace, sex, date of registration and the date issued. The short form is not issued for deceased persons.

Long Form – Ontario Birth Certificate

An Long Form Ontario Birth Certificate is a Certified Copy of the Birth Registration and contains the individual’s name, date of birth, birthplace, sex, location of birth, parents’ names, parents’ date of birth, mother’s mailing address at the time of birth, duration of the pregnancy, number of children born to this mother, parent’s birthplace, weight of the child at birth, type of birth (single, multiple, etc.,), attendant at the birth, date of registration, registration number and the date issued.

You may need a Long Form Ontario Birth Certificate (Certified Copy) if for example:

  • You are moving to another country
  • You are an executor for a foreign estate
  • You are adopting a child abroad
  • You are filling out certain citizenship or immigration documents

Search Letter

A search letter states that according to the Ontario Vital Statistics office the birth is or is not recorded. The exact date of birth is not provided or confirmed.


You will need to provide a guarantor that has know you for at least two years. Guarantor Information is only required for subjects that are age 9 or older. Before completing this application you will need the following information from the guarantor:

  1. Permission by the guarantor to provide his or her information;
  2. Name;
  3. Occupation (see list below of qualified guarantors);
  4. Work address; and
  5. Daytime telephone number.

The following are guarantors for the purposes of section 45.1 of the Vital Statistics Act:

Chief of a band recognized under the Indian Act (Canada), chiropractor, dentist, First Nations police officer, judge, justice of the peace, lawyer, mayor, member of the Legislative Assembly of Ontario (MPP), midwife, minister of religion authorized under provincial law to perform marriages, municipal clerk or treasurer (a member of the Association of Municipal Managers, Clerks and Treasurers of Ontario), notary public, nurse, optometrist, pharmacist, physician, police officer (municipal, provincial, RCMP), principal or vice-principal (primary or secondary school), professional accountant, professional engineer, psychologist, senior administrator (community college or in a CEGEP), senior administrator or professor in a university, signing officer of a bank, caisse d’économie, caisse populaire, credit union or trust company social worker or social service worker, surgeon, teacher in a primary or secondary school or veterinarian
