If you are applying for a birth certificate for a particular agency or organization, it is best to check with them to determine what type of birth certificate they require from you.

Selecting at least one of the three Birth Certificate types (Birth Certificate, Birth Certificate with Parental Information, Certified Copy of Birth Registration) is required.
A birth certificate with parental information is required in order to obtain a passport for a child. Includes the same information as a birth certificate, along with the name of each parent listed on the birth registration and their place of birth. Recommended for children under the age of 16. Also known as the long form or family information birth certificate.
