Northwest Territories Family Information Birth Certificate (Long Form)

The Family Information Birth Certificate is the document of choice for children 16 years of age and under, as it is generally required by government agencies, such as Passport Canada, SGI, Aboriginal Affairs. Includes the individual's full name, date of birth, place of birth, gender, registration number and registration date, PLUS the parents' full names and their province/state and country of birth.

Northwest Territories Individual Information Birth Certificate

The Individual Information Birth Certificate contains the individual’s full name, date of birth, place of birth, gender, registration number and registration date. This certificate may not be ideal for children 16 years of age and under, as many government agencies require the additional information contained on the Family Information certificate.

Photocopy of Northwest Territories Birth Registration

A Photocopy of Northwest Territories Birth Registration is a photocopy of the actual birth registration filled out at the time of birth. This copy is certified as a true copy of the original.

NOTE: Certified copies are rarely needed by citizens and are, by law, for restricted use only. They are generally only required for court or consulate purposes. They are not recommended for use as identification.
