Ontario Birth Certificate Order Form

Birth Certificates are available for order from this website for events that occurred within the Province of Ontario.

If you are obtaining a birth certificate for someone who is deceased, you will need to contact the Registrar's Office at 1-800-461-2156. Be prepared to provide a death certificate or a funeral director's statement of death, and any other documentation requested.

If you are applying for a child and you are not a parent listed on the birth registration you must contact the Registrar's Office at 1-800-461-2156 as adoption documentation may be required.

Guarantor Information (Complete Only if Subject is 9 Years or Older)

IMPORTANT:  Guarantor must be known by the person placing the Birth Certificate order for at least two years, reside in Canada, be a Canadian citizen, is not retired, and is a practicing member in good standing. You must have permission by the guarantor to provide his or her information. The guarantor can be a family member, provided they hold an occupation from the list below. The guarantor does not need to sign the application. Guarantor requirements are in accordance to section 45.1 of the Vital Statistics Act.

IMPORTANT:  No more than one of each type of certificate or certified copy will be issued. All delivery times quoted are average delivery times and cannot be guaranteed. If you are applying for a birth certificate for a particular agency or organization, it is best to check with them to determine what type of birth certificate they require from you.

No Rush Service is available for:

  • Birth Certificate with Parental Information; 3 to 4 week average delivery time.
  • Any other certificates ordered at the same time as a Birth Certificate with Parental Information; 3 to 4 week average delivery time.
  • Newborns: Birth registration takes about 16 weeks. Birth Certificate orders will be processed and delivered only after registration is complete.
  • Births prior to 1930; 6 to 8 week average delivery time.
  • Delivery outside of Canada; 6 to 8 week average delivery time.
