Changes cannot be made once an order has been placed.

If no record of the birth is found by the Saskatchewan Vital Statistics Agency, fees will be applied to the search cost and will not be refunded by the Agency.

Who Can Apply for a Birth Certificate?

  • The person who is the subject of the certificate;
  • The parent, if the subject is less than 18 years old;
  • The custodial guardian, must provide papers;
  • The executor of the estate, must provide the will.

Saskatchewan Family Information Birth Certificate (Long Form)

The Family Information Birth Certificate is the document of choice for children 16 years of age and under, as it is generally required by government agencies, such as Passport Canada, SGI, Aboriginal Affairs. Includes the individual's full name, date of birth, place of birth, sex, registration number, registration date, PLUS the parents' full names and their province/state and country of birth.

Saskatchewan Individual Information Birth Certificate

The Individual Information Birth Certificate contains the individual’s full name, date of birth, place of birth, sex, registration number and registration date. This certificate may not be ideal for children 16 years of age and under, as many government agencies require the additional information contained on the Family Information certificate.

Saskatchewan Certified Copy of Birth Registration

A Saskatchewan Certified Copy of Birth Registration is a copy of the actual birth registration filled out at the time of birth. This copy is certified as true copy of the original.

Saskatchewan Genealogical Photocopy of Registration of Birth

A Saskatchewan Genealogical Photocopy of Registration of Birth is a copy of the birth registration stamped “For Genealogy Only” and is only available for events that took place over 100 years ago.

NOTE: Certified copies are rarely needed by citizens and are, by law, for restricted use only. They are generally only required for court or consulate purposes. They are not recommended for use as identification.
