An Ontario Birth Certificate is your foundation identity document and is used for obtaining a passport, driver’s licence, social insurance number and other legal documents and purposes.
If you are applying for an Ontario Birth Certificate to support a separate application such as for a Passport, Driver's License, Immigration, or Native Status, it is important to check with the organization requesting the document to ensure you obtain the correct vital record. Changes cannot be made once an order has been placed.

Certificates ordered with regular service average 15 to 20 business days to be processed and delivered.

Fees include all taxes, search, shipping and processing charges.
If no record of the birth is found by the Ontario Vital Statistics Agency, fees will be applied to the search cost and will not be refunded by the Agency.
All birth certificates in Ontario are mailed or couriered directly from the Ontario Vital Statistics Agency.
The Ontario Short Form Birth Certificate contains the individual’s name, date of birth, place of birth, sex, certificate number, registration number, registration date, and date issued. This certificate may not be ideal for children 16 years of age and under, as many government agencies require the additional information contained on the Ontario Long Form certificate.
The Ontario Long Form Birth Certificate is a certified copy of the actual birth registration filled out at the time of birth which includes the individual’s name, date of birth, birthplace, sex, location of birth (hospital, birthing centre, etc.), parent's names, parent's date of birth, mother's mailing address at the time of birth, duration of the pregnancy, number of children born to this mother, parent's birthplace, weight, type of birth (single, multiple, etc.), attendant at the birth, date of registration, registration number, and date issued.
